Insurance
Dr. Byrd is a participating provider with numerous insurance plans. Plan types and policy provisions vary greatly from one policy to another and, as the policyholder, it is critical that you are fully aware of the benefits available under your plan.
Before your appointment, please take time to familiarize yourself with your insurance coverage and be sure to bring your insurance card to each visit. If you are a member of a managed care plan that requires a referral from your primary care physician (PCP), we must have this authorization prior to your appointment. Our precertification specialist will contact your insurance company for authorization and benefit information for diagnostic testing such as MRI or CT scans and for surgery. We will submit claims forms on your behalf but are bound by the provisions of your policy. If you have any questions about the requirements of your specific plan, please contact your insurance company. Third party billing (such as liability or motor vehicle accidents) may require special consideration.
Please call our insurance office if you have questions concerning third-party insurance, (615) 284-5800, ext. 11.
